Why there is paperwork
I am one person, working with animals on real terrain, in real homes, and in rooms full of people. Clear paperwork is how I keep your animal safe, keep myself honest about what I can and cannot promise, and make sure nothing important is a surprise later.
Complete whichever agreements apply to the services you are booking. Each one is signed electronically, and a signed PDF lands in your inbox and mine the moment you submit it. Utah law treats your typed name as your signature, the same as ink on paper.
The forms you may need
Pet Care Agreement and Liability Waiver
The master agreement. Covers hiking, pet sitting, drop-in visits, transportation, off-leash time, home access, cancellation, and payment. Everyone signs this one before their first service.
Dog Hiking Service Agreement
For dogs joining Shanti's Pack. Gear and GPS requirements, vaccination status, recall and social fit, trail risk, and the twenty-four hour cancellation policy.
Pet Grooming Agreement
Your dog's grooming profile plus the grooming terms: no dematting, the regular grooming schedule, the pickup window, and the card on file policy.
Veterinary Release Agreement
Permission to seek veterinary care if your animal is hurt or unwell in my care, your treatment limit, and your veterinarian's details. One signature covers future visits.
Sound Bath Waiver
For yoga, Reiki, and sound healing events. Participation, assumption of risk, and an optional photography release.
Sound Bath Waiver, With Your Companion Animal
For Harmony Healing and any sound bath where your animal comes with you. Adds the animal-to-animal risk terms.
The policies worth reading twice
- Twenty-four hour cancellation. Cancel or reschedule at least twenty-four hours ahead. Anything later is charged in full.
- A card stays on file. It is used only for a late cancellation, a missed appointment, or a pickup more than fifteen minutes past the end of a groom. Card details are collected securely and never entered on a form.
- Vaccinations current. Rabies, Bordetella, and Distemper, with tags firmly affixed. Tell me before a visit if anything has lapsed.
- Honest disclosure. Bite history, reactivity, resource guarding, escape attempts, medication, and allergies all need to be on the form. I can work with almost anything I know about, and almost nothing I do not.
- Regular grooming. Booking a groom means committing to a two, four, six, or eight week rhythm. I do not do seasonal grooming with yearly shave downs, because consent to groom comes from the animal as well as from you.
- Veterinary care. If something happens I will reach you or your emergency contact first. If I cannot, I will get your animal to care, and the cost is yours.
